In this section, we provide details on how to enroll members, electronic eligibility and more.

To ensure we continue to meet security and HIPAA standards and safeguard your group's data, we do not accept enrollment information sent directly to the account manager via unencrypted email. The most efficient and secure way to make eligibility changes is directly through Employer Connection. If you need assistance with an issue that cannot be addressed on Employer Connection, please direct your inquiry to eligibility@deltadentalil.com in a secure, encrypted email. If you do not make eligibility changes online, please use the Delta Dental of Illinois Enrollment/Change of Status Form to send changes and send to:

    Delta Dental of Illinois provides welcome kits for your members directly to you either by email or mail. These include an overview of the plan highlight sheet, how to guides for finding a dentist and accessing benefits and claims online and other information on using their dental plan.

    Dental ID cards are mailed to members’ homes; DeltaVision ID cards are generic and available in a PDF version and included with the enrollment kit if applicable.

    Delta Dental PPO Plus Premier
    Dentists will typically need the following information to confirm a member's eligibility:

    • Date of birth
    • Primary member's name
    • Group name

    Members can print a copy of their ID card through Delta Dental of Illinois' Member Connection. Members can also download our Delta Dental mobile app to access an electronic version of their ID card. The app includes the ability to view dental coverage and claims information, obtain a LifeSmile oral health score, find a dentist, and access a cost estimator tool to gauge the cost of common dental treatments in their areas. The Delta Dental mobile app is optimized for iOS and Android devices (smart phones and tablets) and is available for free to download in the Apple App Store or Google Play Store. Simply search for "Delta Dental."    

    DeltaCare

    Members must select a primary DeltaCare dentist. DeltaCare dental facilities receive a monthly roster of DeltaCare patients from Delta Dental of Illinois based on members’ primary dentist selections.

    DeltaCare members can change their primary dentist at any time by calling our customer service department at 800-942-3772. However, all changes must be received prior to the 20th of the month in order for the member's change to be effective for the upcoming month.

    If a member or their dependents make a change after the 20th of the month, the change will take effect in two months.

    If a member needs to see a specialist, their primary dentist must provide a written referral to a DeltaCare network specialist per program guidelines (program guidelines indicate the procedures that can be referred and the protocols associated with such referrals by procedure code). There is an authorization process for both non-emergency and emergency referrals. Members should contact our customer service department at 800-942-3772 to confirm the specialist’s eligibility on any referrals provided.

    DeltaCare ID cards will list the following information:

    • Name
    • Group Number
    • Member ID Number
    • Facility Number

    Vision

    DeltaVision®, in association with the EyeMed Vision Care Access, Select, and Insight networks, offers vision care plans that give enrollees/members access to a national network of both independent providers and leading optical retailers. Members and their covered dependents will receive network benefits when they visit a vision care provider in their designated EyeMed network.

    DeltaVision® ID cards are exclusively available in an electronic version. They are not personalized and all that the vision care provider will need to verify coverage is the primary member's name and date of birth. Download the DeltaVision ID card that corresponds with your group's EyeMed Vision Care network below (available in both English and Spanish).

    Members can access their vision benefit information anytime, anywhere on EyeMed's mobile-friendly member website at eyemedvisioncare.com/member or with EyeMed's free mobile app, which is available to all DeltaVision members. The mobile website and mobile app allow members to access their ID card, view benefit information, and find an in-network vision provider, all while on the go. To download the EyeMed mobile app, visit the Apple App Store or Google Play Store and search for "EyeMed Members."
